What Is A Health Card?
The 'OCL Active' Health Card is designed to increase activity levels and improve health by offering reductions on a wide range of activities for those on low incomes.
Who Is Eligible?
16-18 year olds, over 60s and those who receive benefits identified on the application form (see bottom of page). Dependants To be entitled to a Health Card, dependents that are 16-18 must firstly be accompanied by a parent. They can then apply for the card under parental company. Secondly you must provide a Proof of Child Dependent (s). This can be in the form of:
- A child benefit book
- A Child Benefit Letter from the Benefits Agency Proof that the dependent(s) is the child/children named on the document must also be shown.
When Can You Use The Health Card?
The 'OCL Active' Health card can be used at 'OCL Active' facilities at off peak times from 7am-4pm Mondays to Fridays and all weekends and all swimming lessons. What Activities Are Discounted? 'OCL Active' offers up to a 50% discount on selected activities including gym, swimming, classes, and bowls. See website for further information. How much does it cost? The cost of a card is £3.00 for 6 months. Under 16s do not require a card as they attract the discount automatically. Alternatively, memberships are available from £15 per month (off peak)
How to Apply:
1. Pick up an application form from one of our Centres at the reception and complete the necessary information.
2. Take the form, along with proof of your eligibility and payment, to any of the following 'OCL Active' sites: Chadderton, Crompton, Failsworth, Glodwick, Oldham, Royton, Saddleworth.
3. Payment can be made by cash, cheque or credit card. Cheques made payable to Oldham Community Leisure Ltd.
4. A member of staff will take your form and payment and check proof of entitlement. You will be issued with a receipt and your health card.
5. If you lose your card it can be replaced at a cost of £2.00.
6. Cards are issued for 6 months and can be renewed for as long as you are eligible.
Conditions of use:
1. Use of the health card is restricted to off peak times with the sole exception of swimming lessons.
2. Where places are limited, some exercise classes must be booked in advance. Failure to attend once booked will result in that activity becoming chargeable at full price.
3. Present your card at the reception on each visit to be granted admission.
4. The safety of non-swimmers and children is the responsibility of the supervising adult who is to be in attendance at all times. The supervising adult must maintain a constant watch over the children and be in close contact with those that are under the age of 8, or are weak or non-swimmers.
All children under the age of 8 must be accompanied in the water by a parent or responsible adult (aged 18 years and over). An adult cannot be responsible for supervising more than two children under the age of 8.
All non-swimmers must wear a floatation device unless closely supervised on a one-to-one basis by an adult who can swim*. All non swimmers and those wearing floatation devices are restricted to the designated shallow areas at all times and are not permitted to enter water of a depth greater than 1.35m.
5. The 'OCL Active' Health Card is non-transferable.
6. Misuse of the Card or facilities will result in the forfeiture of the Card.
7. 'OCL Active' reserves the right to amend the scheme and regulations without prior notice.
